To apply for a VITAL Card, you must be an 18-or-older legal US resident with a:
US phone number and smartphone (both iOS and Android are supported)
Physical US mailing address (PO boxes are not accepted)
Social Security Number
US Bank account (bank services such as Venmo, Cash App, and Apple Pay are not applicable)
Valid ID (driver’s license, passport book, or passport card)
